SEMP Surveys are available on the ISS Portal:

SEMP surveys can be accessed directly from your ISS Portal homepage. From your homepage, click on the “Go to SEMP Survey” link and you will automatically be taken to your SEMP Survey page.  Since the best and easiest way to get to the SEMP Survey is via your Portal, we have removed the link directly to it from our website. In order to get started, go to the ISS Portal at https://portal.issny.org/
